Our mission Almost Home Kids provides transitional care in a home-like setting to children with complicated health needs, training for their families and respite care. A bridge from hospital to home AHK provides a Transitional Care Program for children who are ready to be discharged from a hospital but for whom preparations at home need to be made. In a comforting environment, children receive 24-hour medical and nursing support from skilled pediatric nurses. AHK trains families on how to both care for their children who are medically fragile and prepare the home for their child. Respite care The Respite Care Program at AHK offers children a comfortable place to stay with access to medical and nursing support while their parents/caregivers tend to personal or family issues. The Respite Transportation Program provides roundtrip transportation for a child from their home to an AHK facility.
